STUDY 1992 HTM file
Disulfiram treatment of alcoholism

Chick J., Gough K., Falkowski W. et al.
British Journal of Psychiatry: 1992, 161, p. 84–89.
Still relevant today, from the early 1990s a UK randomised trial of disulfiram in the treatment of alcohol dependence found that it significantly reduced drinking when there was daily supervision to make sure patients took the tablets, and they knew these would cause unpleasant physical effects if they drank.

REVIEW 2010 HTM file
Acamprosate for alcohol dependence

Rösner S., Hackl-Herrwerth A., Leucht S. et al.
Cochrane Database of Systematic Reviews: 2010, 9. Art. No. CD004332.
The first review from the prestigious Cochrane collaboration of the drug acamprosate as a treatment for alcohol dependence finds clear evidence that it offers worthwhile if modest benefits in preventing a return to drinking after detoxification.

Alcohol Treatment Matrix cell E2: Treatment systems; Generic and cross-cutting issues

Ashton M.
Seminal and key studies on local, regional and national systems for effectively and cost-effectively providing treatment. Explores whether payment by results stifles patient-centred practice or stretches services beyond comfort zones, the surprising results of a randomised trial of service-improvement mechanisms, and the multiple answers to how many drinkers should be in treatment. See the remaining four cells in row 2 of the matrix for more on generic features of medical and psychosocial therapies.

HOT TOPIC 2020 HTM file
How many drinkers should be in treatment?

Ashton M.
One of our selection of hot topics – important issues which sometimes generate heated debate over the facts or their interpretation. What proportion of England’s problem drinkers might be in treatment, how far does that fall short of the number who should be – and how do we judge ‘should’? The proportion varies to a surprising degree depending on assumptions about who needs treatment.

Effectiveness Bank Alcohol Treatment Matrix cell D2: Organisational functioning; Generic and cross-cutting issues

Ashton M.
Key studies on the how the characteristics and functioning of treatment organisations affect implementation and effectiveness. Learn to see organisational context as part of treatment and about two evidence-based US quality improvement resources, and consider what makes treatment services engaging and how they could extend engagement into long-term continuing care. See the remaining four cells in row 2 of the matrix for more on generic features of medical and psychosocial therapies.

STUDY 1992 HTM file
Health promotion in the general practice consultation: a minute makes a difference

Wilson A., McDonald P., Hayes L. et al.
BMJ: 1992, 304, p. 227–230.
‘A minute makes a difference in primary care consultations’, was the finding of a 1992 study about improving the capacity of general practitioners to screen for problems such as heavy drinking, smoking, and high blood pressure. But is extra time on the clock enough to secure routine (as opposed to more frequent) delivery of health promotion and brief intervention?

Alcohol Treatment Matrix cell B2: Practitioners; Generic and cross-cutting issues

Ashton M.
At the front line the practitioner is to the patient the face of treatment. They can matter enormously – not so much in their formal credentials, but their manner with patients. Tour seminal and key studies which probe the heart of addiction treatment: relationships. See the remaining four cells in row 2 of the matrix for more on generic features of medical and psychosocial therapies.

MATRIX CELL 2020 HTM file
Alcohol Treatment Matrix cell A2: Interventions; Generic and cross-cutting issues

Ashton M.
Seminal and key studies on aspects of the treatment of problem drinking relevant both to psychosocial and medical approaches. Focuses on ‘common factors’ often sidelined as components of the ‘placebo effect’, but which are actually active ingredients now recognised as at least as important as the particular therapy. See the remaining four cells in row 2 of the matrix for more on generic features of medical and psychosocial therapies.

STUDY 2015 HTM file
The impact of paying treatment providers for outcomes: difference-in-differences analysis of the ‘payment by results for drugs recovery’ pilot

Mason T., Sutton M., Whittaker W. et al
Addiction: 2015, 110(7), p. 1120–1128.
A flagship drug treatment policy initiative appears to have backfired in England, where the government’s pilot payment-by-results schemes seem to have led to fewer successful completions of treatment and more prospective patients declining treatment.

HOT TOPIC 2020 HTM file
What about evidence-based commissioning?

Ashton M.
‘Hot topics’ offer background and analysis on important issues which sometimes generate heated debate. In the UK the landscape of drug and alcohol service commissioning to meet need in an area has radically altered – a leap in the dark illuminated only dimly by prior evidence and current evaluations.
