This unique randomised trial tested what would happen if detoxified opiate addicts were then maintained on a substitute drug, on an opiate-blocking medication, or simply counselled. The results led to the introduction of methadone prescribing programmes in Malaysia.

Improving clinical outcomes in treating heroin dependence: randomized, controlled trial of oral or implant naltrexone
The first trial of implanted versus oral naltrexone found that the implants' extended opiate-blocking action helps avoid relapse to regular opiate use – but the action was not as extended as hoped, non-opiate use was greater, and there were more unpleasant side-effects.

Antidepressants curb depression but add little to strong 'talking therapies'
A trio of reviews of trials of antidepressants in the treatment of depressed alcohol or drug dependent patients have clarified that they do help the severely depressed, but also that they add little to psychosocial approaches such as cognitive-behavioural therapy.
About a third of Britain's heroin dependent patients also have problems with crack cocaine. Can opiate substitute prescribing help with both problems, and which special anti-cocaine therapies are worth adding on? This review trawled the international research for the answers.

1 in a 100 chance of dying after treatment with heroin-blocking drug
In Australia heroin-addicted patients trying to avoid relapse by taking the opiate-blocking drug naltrexone had at least a 1 in a 100 chance of dying within three months, usually from overdose in the weeks after treatment ended; the true figure may have been as high as 8 in a 100.

Arousing visceral opposition and passionate defence, prescribing opiate-type drugs for as long as needed has for decades been the mainstay of heroin addiction treatment in Britain. With the weight of government behind them, that position was challenged by ‘recovery’ advocates; in 2012 an expert report sought to reconcile the competing perspectives.
