Seminal and key studies on management and supervision in psychosocial therapies. Focus is on evidence of the need for post-training ‘coaching’ and for letting therapists know how their clients are doing – especially when they are doing badly.
Instead of a set programme, a clinic in London tried offering methadone or buprenorphine patients still using heroin or cocaine a selection from a suite of well-supported psychological interventions tailored to the patient and then systematically re-tailored in the light of how they responded. It worked – but did it work well enough, and would the findings be replicated in more typical circumstances?

Randomized controlled trial of harm reduction treatment for alcohol (HaRT-A) for people experiencing homelessness and alcohol use disorder
Heavy drinking is clearly problematic for homeless populations, but is the best way to tackle it to aim for abstinence, or to accept the reality of life on the streets and aim to reduce harm and improve lives in ways which make sense to the patient? This US study supports the latter, but without conclusively deciding the issue.

A meta-analysis of the association between patients’ early perception of treatment credibility and their posttreatment outcomes
Review commissioned by the American Psychological Association suggests therapists should incorporate strategies to enhance the credibility of treatment when explaining its rationale and throughout therapy. The more a treatment ‘makes sense’ to a patient, the better outcomes tend to be.

A meta-analysis of the association between patients’ early treatment outcome expectation and their posttreatment outcomes
Review commissioned by the American Psychological Association found that clients who start psychotherapy expecting good outcomes tend actually to have better outcomes than other clients, raising the issue of whether therapists should regularly assess expectations and if needed take steps to enhance them.

Meta-analyses of the relation of goal consensus and collaboration to psychotherapy outcome
Research findings amalgamated for the American Psychological Association show that outcomes are substantially better the more clients and therapists agree on goals and methods and form collaborative working relationships to implement those agreements. The findings support engaging patients as partners in setting treatment goals and methods.
