The annual accounting of the treatment caseload in England registers a continuing fall in total numbers and decreasing success with opiate users, while success with drinkers has increased and has for the last few years remained relatively high and stable. An ageing population of opiate users is the proposed explanation for the former trend – but why hasn’t a similarly ageing alcohol caseload also eroded success rates?

A double blind, placebo controlled trial of modafinil for the treatment of cocaine dependence without co-morbid alcohol dependence
Modafinil is a mild stimulant which it is hoped might plug the gap in effective pharmacotherapies for dependence on cocaine. Though this trial found that it promoted abstinence from cocaine, others have not, and its inconsistent benefits have been seen as failing to outweigh the drug’s side effects.

The limitations of US health insurance permitted this rare quasi-random test of whether 12-step treatment is effective for adolescents dependent mainly on cannabis or alcohol and of whether doing this in a residential setting improves outcomes.

Holistic family therapy preferable to less comprehensive therapy for troubled teens
Family therapy orchestrating multiple dimensions of a child's life was more effective than group child or family approaches for US teen drug users. Extended text highlights main advantage – effects persist while relapse is the norm after other therapies.
It worked as well as somewhat longer and more elaborate experimental therapies, but how would a basic US programme for cannabis using youngsters fare when compared to much more extensive real-world therapies? On average at least as well if not better was the answer.

Patient numbers have fallen in line with trends among young adult patients and the general population, a sign of fewer drug problems generally, though cannabis bucked the downward trend.

Long-term effect of community-based treatment: evidence from the adolescent outcomes project
The title speaks of long-term effects but in fact there were none from sending young US substance users to a youth therapeutic community specialising in substance use problems compared to non-specialist group homes; early gains had all eroded, an instance of the general difficulty of sustaining youth treatment outcomes.
